Enigmatic Wrist Doctor with New Contemplative Techniques

Forearm is connected to the hand:

By wrist joint which is a synovial joint of the upper limb. It comprises of eight carpal bones and the radius and ulna bone. Wrist is the main bridge between the hand and the forearm and works articulately to maintain stability of actions which is contributed by the framework of joint capsule and ligaments.

Movements in the wrist joint:

Are executed by the muscles of the forearm. It is the wrist that works as a transition between the hand and the forearm and any disorder or injury that affects the wrist debilitates movement of the hand limb thus affecting everyday activities. Emphatic treatment by the wrist doctor helps to ensure safety against long term disability.

Stress and sudden injury:

Are not the only conditions that might be treated by the wrist doctor. Osteoarthritis, Rheumatoid Arthritis, fracture of the wrist bones, pressure on the median nerve, Ganglion Cysts, Keinbock’s diseases are some common wrist problems which may cause varying degree of pain and limitations and require immediate medical attention.

Application of Metal Implants

Wrist joint is the synovial joint which acts as a bridge between the hand and the forearm. Also known as radiocarpal joint, the wrist joint is made up of eight carpal bones, four ligaments, three nerves, joint capsule and muscles which provide stability and facilitate movement in the wrist.

There are various injuries that cause break in the wrist bones, termed as wrist fractures which can affect the mobility and the functionality of the wrist indirectly interfering with the activities performed by the hand and the forearm, leading to partial or total handicap.

Rationale:

Fractures in the wrist may occur due to fall on an outstretched hand or collision, while playing racquet sports or when the bones have become weak or arthritic. Intense pain, swelling and deformity in the wrist caused by the fracture limits its functional movement and it is essential to determine the nature and severity of the fracture.

Approach considerations:

After the wrist doctor finds conclusive evidence that the pain and swelling is a result of fracture, the technique is decided upon after evaluating factors that are-

  • - The age, health and activity level of the patient
  • - The nature and severity of the fracture
  • - Presence of associated injuries

Techniques:

The treatment techniques that the wrist doctor adopts for fixation of fracture are implemented after an in depth critical analysis of the nature and severity of it. The techniques undertaken by the wrist doctor are based on the principle of realignment - fixation - immobilization.

Casting / Splinting:

The wrist doctor places the wrist in a cast or splint to protect the broken bones and stabilizing them to enable healing. Casting or Splinting are done for a definite period of time and then removed after the wrist doctor is reassured that the bones have healed. They involve covering of the fractured bone with two layers of bandage that are soft on the inner and hard on the outer base.

Closed Reduction:

Under closed reduction technique the broken bones are physically realigned by manipulation without exposing the bone surface. The wrist is thereafter placed in a cast or splint after reduction. Sometimes the wrist doctor further holds the bones together with fixators.

Metal Implants Fixation Technique:

In some cases just realignment and reduction are not sufficient even after placing them in a cast or splint. They require stabilization by implants such as surgical pins or screws, to be held together. The fixators that are used by the wrist doctor internally or externally are-

Plates -

Type of internal splint which hold bones together by being attached to them with screws.

Screws -

The most used implant for internal fixation that come in various sizes for different bones. They are either used as an individual implant or as support with plates, rods or nails.

Nails / Rods -

Used as implant to fix long bones. They are inserted through the hollow centre of the bone and then held with screws on both ends to stabilize it.

Wire / Pins -

Used as internal fixators for very small bones that cannot be held together by screws.

Complications:

When the wrist is placed in a cast or splint, the wrist tends to become stiff and it is only with an effective implementation of rehabilitative program that the stiffness is reduced.  The reduction and fixation technique may result in failure to heal the bones or the bones may heal with a deformity.  Infection is also another common complication that fixation by metal implant is prone to.

Benefits:

The techniques of closed reduction with metal implant application and placing the wrist in a cast or splint are the most accurate technique for mild fractures. They are relatively less painful in comparison to an open surgery and the patient is also saved from the anxieties related to surgical alignment.

Rehabilitation:

It is important that the patient follows a well formulated rehabilitation regime under the guidance of a skilled therapist to restore mobility in his hand and reduce the persistent stiffness that is prevalent in the wrist because of a long period of immobilization. The program comprises of protocols that enable the patient not only to learn pain management but also gradually restore functionality with massages, gentle exercises and other tools.
